docs: clarify councillor model configuration

The documentation incorrectly implied that the councillor model could be
configured via , but the model is actually
always taken from the preset configuration ().

Changes:
- Remove  from the  example since it's
  ignored (model comes from preset)
- Clarify that  configures temperature, MCPs, skills
  but NOT the model
- Update the Constraints section to be accurate about what can be
  configured via
- Add note that the default model is only a fallback

Fixes #375
