feat: Add full dev-shell-tools permissions to /init-perms

- Include all 21 tools from dev-shell-tools repo
- Add rg, fd, fzf, zoxide, broot, ast-grep, bat, eza, delta, difft
- Add lazygit, gh, tokei, sd, procs, hyperfine, http
- Update template and command documentation
- Add --full template with cloud/container tools (docker, kubectl, terraform, aws, etc.)
