docs(skills): fleetflow default posture — verify-by-default, scale-to-ask, wave throttling

The native tool's fan-outs are effectively automatic because its doctrine
makes them the script-author's default (30-50 agents on large runs is the
pattern working). Encode the same posture: every run gets a verify phase
unless stated otherwise; scale lanes to the ask; throttle in waves of 4-6
per provider without shrinking the plan; no silent caps.
